What should I upload for 'Articles of Incorporation'?

Upload a document issued by the relevant Secretary of State showing your business name, state of formation, and incorporation ID.

What is a 'Formal Document of Ownership'?

A formal document of ownership could be an Operating Agreement for LLC, a Stock Certificate for public companies, or a letter from the Secretary of State for corporations. Essentially, it is a legal document that declares who owns the company, that has been submitted to the relevant authorities.
